What is Qubbs?

Qubbs is a Scheduled ED Medication that is used in conjunction with a 3rd- or 4th-line medical regimen. It is sometimes used in combination with other Meds to improve the quality of life for patients with health problems that may affect their physical, mental or relationships. Qubbs may be used for purposes that are not listed in our full prescription list.

Orlistat has been shown to cause mild side effects in some individuals. Talk to your health care provider if these do not go away within a few days. If you begin to experience more serious effects, seek medical attention immediately.

Common side effects of orlistat use include:

  • Gas/ flatulence
  • Stomach pain
  • Abdominal pain
  • Diarrhea
  • Increased number of bowel movements
  • Urgent need for a bowel movement
  • Oily stools
  • Oily spotting on underwear and clothes
  • Loss of appetite
  • Headache
  • Nervousness/ anxiety

More serious side effects include:

Orlistat has been shown to affect the absorption of fat-soluble vitamins (vitamin A, D, E and K). This can prevent individuals taking this medication from receiving valuable nutrients from their food. Your doctor may recommend that you take a multivitamin and beta-carotene supplement along with this medication to ensure that you are receiving adequate nutrition from the food you eat.

As with all medication, inform your health care provider of any medical conditions you are currently managing. Let them know if you have or ever had kidney stones, diabetes, liver disease, gallbladder disease, thyroid disease, or an eating disorder (anorexia or bulimia). In addition, you should tell them if you have ever had an organ transplant, cholestasis (reduced or stopped bile flow), or malabsorption syndrome.

Tell your provider about any and all medication, prescription drugs, and supplements you are taking before starting treatment with orlistat. Orlistat can interact with substances in the body, causing potentially serious adverse reactions. Specifically, you should inform them of any immunosuppressant medication you are currently taking, such as cyclosporine. You should also let your provider know if you are currently taking blood thinners–like warfarin–or diabetes medication–like metformin.

In addition, let your doctor know if you are planning to become pregnant, are pregnant, or are currently breastfeeding before starting treatment with this medication.

Indications

Adults: Orlistat 120mg 120-80mg (120-80mg is the recommended starting dose in adults with a body mass index (BMI) of 30 or more kg/m2, weight-based treatment, usually with a reduced-calorie diet, physical activity and other dietary changes. If the patient has a BMI over 30 kg/m2, a reduced-calorie diet, regular physical activity and other non-dietary changes are not suitable, the dose may be increased and should be reduced.

Patients with severe obesity (BMI greater than 30 kg/m2) should have weight- and adipose tissue biopsy (X-ray or tissue biopsy) performed before initiating treatment with orlistat. The patient should have a normal physical examination, laboratory and medical history and have normal physical examinations and physical exercise and dietary advice before initiating treatment. If clinically appropriate for the indication, the patient should have a physical examination and/or laboratory report including laboratory values, body composition, blood pressure and heart rate.

Patients with mild to moderate hepatic impairment should have a reduced-calorie diet, regular physical activity and/or dietary changes. Patients with severely impaired hepatic function should have a normal-calorie diet and/or an appropriate treatment plan. Other interventions (e.g. nutritional support and therapy) may be required in severe hepatic impairment.

Patients with a BMI over 27 kg/m2 or severe hepatic impairment and the following conditions may benefit from reducing-calorie diet initiation:

  • severe renal impairment (creatinine clearance less than 30 mL/min)
  • liver disease
  • persistent diarrhoea
  • passing less than 2 fg/day (2-3 g/day), flatulence
  • diarrhoea with or without mucus discharge
  • anorexia
  • anorexia nervosa
  • seizures or dementia
  • severe anorexia nervosa
  • severe preexisting systemic lupus erythematosus
  • severe familial hypercholesterolemia or familial hypercholesterolemia, a history of high cholesterol
  • chronic obstructive pulmonary disease (COPD)
  • history of coronary artery disease
  • tobacco use
  • hearing loss
  • history of gallbladder disease
  • history of fatty liver disease
  • history of pancreatitis
  • history of pancreatitis, pancreatitis associated with gallbladder disease
  • liver dysfunction
  • irregular menstrual periods
  • alcohol/dietary changes
  • hypercholesterolemia (low-density lipoprotein cholesterol) with or without hypertriglyceridemia

In some patients with hepatic impairment, the dose of orlistat may be reduced to reduce the risk of gastrointestinal (colitis) and/or abdominal pain, e.g. in patients with chronic kidney disease or cirrhosis. In these cases, the patient should have a regular clinical and laboratory assessment and should be given appropriate advice and support.

Children and adolescents: The dose of orlistat in children is typically reduced to 120mg daily, whilst the dose may be increased to 80mg daily if the BMI is greater than 30kg/m2.

In the elderly, orlistat is usually recommended in patients with a body mass index (BMI) of 30kg/m2, weight-based treatment, usually with reduced-calorie diets and physical activity. In this patient group, the usual starting dose is 80mg daily.

In patients with hepatic impairment, orlistat is usually recommended in patients with severe hepatic impairment.

Varying doses for weight loss

Most weight loss drugs work by helping to block the action of certain chemicals in the body called fat stores. These storage chemicals then flow into the absorption of food. Xenical decreases the amount of energy a person leaves the stomach and otherbig toe fat. This aids them in suppressing their weight. The recommended doses for 60/IUI are 120mg and 60/IUI. However, the dose for 120/40mg may be higher or lower depending on the size of the Soft Tabs we take.
